Please forgive me, God
God, please forgive me for my sins.
I will try not to repeat them again.
When I get very angry, I cuss and sometimes I take your name in vein.
I am really sorry for saying words that are profane.
Please forgive me and please forgive other people too.
If anybody is capable of forgiveness, it is certainly you.

Using the Lord's name as a cuss word isn't the only way to take it in vein.  Being a Christian and not behaving as one is also taking His name in vein and is every bit as damaging.  But you are right... He is capable of forgiving if we repent.
Good prayer poem
